In the lead up to this particular scene, Carter Horton says he doesn't fear death and will go out his way to decide how he dies, not Death.
    He parks the car on the train track with an incoming train on the way, with the lights and warning sounds ringing and barrier brought down.

    He's eventually convinced by Alex to not do this and as he tries to start the car, it doesn't start at all, the engine keeps failing. He decides to undo his seatbelt and open the door but the seatbelt is stuck and the door won't open at all.

    Eventually Alex saves him but pulling him out of the trapped seat and barely surviving the train crashing into the car but it's later shown very subtly, though never addressed or mentioned at all that the brave fearless Carter Horton was scared and wet himself.
